My understanding is it depends on how much you use it. Keep it functional and it wont shrink too much (depending on which blocker you use), ignore it completely and it will shrink a good deal and even atrophy. The latter is from personal experience. By the time i was made aware i was supposed to be using it, I was already suffering pain just from urination, due to to atrophying tissue. Was honestly happy to lose the previous size.

It only shrinks if you’re not using it 😉
