Sword: Paineater
Main effect: Melee Damage increases by 1% for each 2% decrased health.
Bonus effect: 10% critical Hit chance
Gem: Perfect warrior gem: 30% increased damage with this weapon.
How to get it: killing epic orcs or death threat seems to be the best way.

Dagger: Raiders dagger
Main effect: Enemies killed by stealth attack have 40% increased chance of dropping Mirian
Bonus effect: Critical damage is increased by 33% (counts on all weapons)
Set bonus: picking up Mirian Increases damage by 2% per mirian gained for 20 seconds.

How to get it: kill a legendary marauder assassin or trickster

Main effect: Ranged damage dealt increases by 33% when health is below 35%

How to get it: Kill a legendary marauder tank or destroyer
Gem: Perfect warrior gem: 15% increased damage with all attacks

Cape: Mask of undying
Main effecet: Water of Lorien and Spectral Dash cost no focus. Melee and stealth damage will increase by 33% for 5 seconds after use.
Bonus effect: Undead allies take 50% less damage (cant be reforged)
Gem: Perfect warrior gem: 15% increased damage with all attacks
How to get: completing the last story sieges

Main effect: damage dealt increases by 15% when Health is below 35%
Bonus effect: elven rage last 33% longer
Set bonus: Perform attacks reguiring Might by comsuming 40% of you max health instead
